Thank you for your interest in the Champions for Children program. In addition to providing direct financial assistance to families and children with special needs, a desired outcome is to raise awareness of resources and services offered by non-profits in Georgia. At the bottom of this page you will see the link to download the application. Directly above the application link is a link to a 6 question survey. Please complete the survey as part of the application and mail it along with the required documentation to the following address:

After the Champions Coordinator reviews your application and determines that you are eligible for the Champions for Children program, your application and contact information will be forwarded to the Easter Seals affiliate in your area. Your local Champions Coordinator will then contact you by mail to inform you that your application has been approved and will instruct you on how to begin receiving services.
